Consumer Choice #3 – Total Health**
This program option includes all the wonderful services that are included in both, the Dental Plus and the Basic Health programs. These services include Dental, Vision, Prescription, Chiropractic, Physician, Ancillary, Hospital and NurseLineSM for a low monthly cost of $39.95.

Consumer Choice #4 - Total Health Plus**
This program option includes ALL the great services listed above, along with additional Non-Medical programs for you and your employees. These programs include Dental, Vision, Prescription, Chiropractic, Physician, Ancillary, Hospital and NurseLineSM, CLC Identity Theft Assistance, CLC Legal and United States Auto Club - Motoring Division for a low monthly cost of $59.95.

*All Dental Plus memberships require a $20.00 one time registration fee.

Since it is not health insurance, members are responsible for payment at point of service of discount healthcare fees incurred. If you currently have insurance, you should be aware that canceling that policy may make it difficult to later obtain coverage. AmeriPlan Health® recommends that you speak to your insurance agent or your company's insurance administrator before canceling any health insurance policy for any reason.

The average savings on the procedures stated in this brochure are only a sample from the discount fee schedule that each AmeriPlan Health® provider has agreed to honor for Members. The discounts represent a reduction on a treatment or percentage basis off the usual and customary fees charged by providers for a specific treatment.
